We know you are excited to anticipate the birth of your child and we are looking forward to providing your care.

In order to facilitate your visit, you will need to bring the following to your first OB appointment:

• All paperwork that was mailed to you from our office or click here for new OB patient forms
• All medical records you may have from another doctor, especially if seen during this pregnancy
• Insurance card
• DVD-R/CD-R to record your sonogram

Your first OB visit may last up to 2 hours for the examination, insurance consultation, bloodwork and sonogram.
